In order to earn a Circles of Excellence Bonus Point, submit proof and a 1-paragraph description of the mentor's activity in your clasroom.
- For a Fall Bonus Point, this form must be submitted by December 31 at 11:55 PM ET.
- For a Spring Bonus Point, this form must be submitted by May 31 at 11:55 PM ET.
This form must be submitted by the deadlines above to receive a point. Submissions are accepted throughout the school year.

Firms that have already earned a Fall Bonus Point for this parameter are eligible for a Spring Bonus Point if the second mentor's activity occurred from Jan 1 - May 31.
